What to expect
Your health plan determines an assessment is needed
For a variety of reasons, your health plan identifies that an in-home assessment would be beneficial or needed and submits a request through the Periscope HIPAA-compliant portal.

A Periscope clinician reaches out to schedule
A licensed PT, OT or RN will contact you at the number provided by your health plan to schedule your visit at a time convenient for you.
The in-home assessment takes place
Your Periscope clinician visits your home to observe, ask questions and understand your needs and environment. You may be asked to demonstrate certain daily activities. Photos or videos may be taken with your permission.
Findings are submitted to your health plan
A comprehensive clinical summary is securely submitted directly to your health plan. Periscope’s role in the process ends here.
Your health plan determines next steps
Your health plan reviews the Periscope assessment and determines the next steps.
