A Periscope clinician will visit your home to better understand your needs, your environment and how to best support your care.

Your health plan determines an assessment is needed

For a variety of reasons, your health plan identifies that an in-home assessment would be beneficial or needed and submits a request through the Periscope HIPAA-compliant portal.

A Periscope clinician reaches out to schedule

A licensed PT, OT or RN will contact you at the number provided by your health plan to schedule your visit at a time convenient for you.

The in-home assessment takes place

Your Periscope clinician visits your home to observe, ask questions and understand your needs and environment. You may be asked to demonstrate certain daily activities. Photos or videos may be taken with your permission.

Your health plan has partnered with Periscope to better understand your needs and ensure you receive the most appropriate care, equipment or services. The assessment is conducted by a licensed clinician and helps gather accurate, objective information about how you move and function in your daily environment.
